THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. |

1. We will continue to communicate with other parties as needed in order to provide you with quality medical care. These are medical care providers who referred you to this office, including providers to whom you have agreed to be referred, your insurance company (for treatment and billing purposes) and laboratories and pharmacies who participate in your care. Unless you instruct us otherwise, we will continue to exchange information with these parties. We may send appointment reminders to you by phone or mail, and may leave messages on your answering machine. We may contact you about other details of your medical care. If you send us a postcard, it might be put on a bulletin board where someone could read it. If you want your information handled any other way, please tell us in writing. 2. You will need to give express, written authorization before the exchange of information between us and anyone besides the parties noted above. There will be no other information released from your medical record outside this office, except when such exchange is required by law or court order. You have the right to request restrictions on certain uses and disclosures of your information. We might not be bound by your request. 3. Your medical information belongs to you. You have the right to inspect and receive a copy of the record of your medical care in this office in a confidential manner. The physical record belongs to us and, in fact, we are the legal custodian of the record of your medical care here. There will be a charge for fulfilling your copying and mailing requests. If you believe our chart is not accurate, please put your concerns in writing and give them to us. You have the right to request a list of those with whom we have shared your medical information. 4. Our office staff has been trained to comply with these privacy provisions. Notify us if you wish to speak with the privacy officer. 5. Our security precautions include the locks on the door protecting your physical record and our computer files. Only our staff and the employees of the landlord may lawfully enter the office when we are not here. When we are here, your record and our computer terminals are observed constantly to prevent unauthorized persons from viewing your information. Our computer files are not accessible electronically from outside the office. Our computer files are backed up periodically to prevent loss of data, and the backup media are stored in a secure place. Otherwise computer hardware and software containing your information does not leave our office. 6. Paper information with your name on it which is to be discarded is destroyed in a confidential manner. 7. This policy may be updated from time to time. A copy of the this policy and any updated policy will be available upon your request.
